Meals on Wheels Walkathon

The 2013 Meals on Wheels Walkathon was a great success! Over 500 participants raised nearly $75,000 for the Meals on Wheels Program on Saturday, May 4th at the Franklin County Technical School track, all while having fun in the sun. Live performers entertained the walkers as they walked, ran, and even biked around the track. Seen here are two walkers from Team Dot.

FCHCC Purchase Service Contracts FY 2013-2015

Franklin County Home Care Corporation has a formal procurement process for the awarding of State Home Care contracts. The procurement process is conducted once every three fiscal years. FCHCC is currently seeking Proposals from agencies for the provision of services in accordance with 651 CMR 3.00 in Franklin County and the Worcester County Towns of Athol, Phillipston, Royalston and Petersham for the following Non-Homemaker services: Habilitation Therapy, Behavioral Health Services, Home Health Services, Companion, Alzheimer’s Day Program, Emergency Shelter, Wanderer Locator Service, Chore, Transportation, Laundry, Grocery Shopping and Delivery, Medication Dispensing System, Personal Emergency Response System (PERS), Enhanced PERS, Environmental Accessibility Adaptations, Adult Day Health, Translation/Interpreting Service, Legal Services, Competency Evaluation, Respite Care, Supportive Day Program, and Vision Rehabilitation.

*Please note that Homemaker/Personal Care Homemaker and Supportive Home Care Aide services require completion of a separate set of application materials. The Homemaker Notice of Intent (NOI), an online application process, is posted under the “For Professionals” link at www.800ageinfo.com by the Executive Office of Elder Affairs. Personal Care/Homemaker agencies also need to fill out Section VIII only of the State Home Care Application for EACH Non-Homemaker service you are proposing in addition to the online NOI application.

AWARD OF CONTRACTS

FCHCC reserves the right to accept or reject any or all proposals. Agencies chosen to provide services will be required to enter into a written Provider Agreement with FCHCC. Examples of this contract are available upon request. Entering into a contract with FCHCC does not guarantee a level of service to be provided. FCHCC is responsible for the determination of client eligibility and level of service.

Contracts will be awarded to successful applicants from October 1, 2012 – September 30, 2015 with annual renewals.
